This one-hour appointment includes:

• 30 minutes with a registered nurse

• 30 minutes with a qualified GP

You're eligible for a free, bulk-billed annual health assessment funded by the Australian Government.

• Physical health: Including blood pressure checks, mobility assessments, and a review of your overall physical condition.

• Mental well-being: Assessing your mood, cognitive function, and social support networks.

During your assessment, a caring and experienced nurse will:

• Assess your functional abilities: Evaluating your ability to perform daily activities independently.

• Provide personalised recommendations: You'll receive a detailed report with recommendations for maintaining your health and well-being.

• Coordinate with your GP: Your nurse will collaborate with your doctor to ensure you receive any necessary follow-up care.
